TERMS AND CONDITIONS OF USE OF THE HUMA MEMBERSHIP CARD:

 

• Only people over 15 years of age can participate.

 

• Upon purchasing a card, the card holder will be obliged to fill in and submit a questionnaire concerning his or her physical condition and provide information on any current medical issues or course of medication.

 

•  No refund is offered for card cancellation.

 

• In case of renovations or an upgrading of our establishments or part of them, the company reserves the right to, during that time, close down either the whole establishment of part or it. Prior notification shall be given.

 

• To allow us to provide an improved service to all our customers, early notice of cancellation, at least 3 hours prior to a scheduled appointment, is necessary. In the case of missing a scheduled appointment, without appropriate prior notification, a charge of 50% of the cost of the service will be applied.

 

• The card is personal, non-transferable, and cannot be frozen or extended.
